What types of services are offered through your office?

Billing and InsuranceWe provide treatment for almost all types of orthopaedic and sports-related injuries. This includes shoulder, elbow, knee, hip, ankle, and foot injuries, as well as treatment of fractures, sprains and overuse injuries. Likewise, we provide expert care for arthritis, which may include total joint replacement.  Please be aware that in addition to the charges for seeing your provider, you may receive a bill for other services we provide directly, such as x-rays, injections, braces, surgical and rehabilitation services. Do I need a referral to see the doctors there?

We do not require a referral. However, since many insurance plans require referrals to specialists, we recommend you contact your insurance company to get specific information on your particular requirements.

What insurance plans are accepted?

We work with Blue Cross, HealthPartners, Medica, Medicare, Medicaid, UCare, Tricare, Humana—Senior Plus and Workers Compensation to name a few. If your insurance company is not listed, feel free to contact our Billing Department at 952.946.9777 for more information.

What if workers' compensation is covering my visit? What do I need to bring to the appointment?

If you are seeing us for a workers' compensation related injury, we need the following information before we can see you:

  • Workers' Compensation insurance company Name and Billing address
  • Claim Number
  • Date of Injury
  • Claim Adjuster Name, Phone number, and Fax number
  • Employee Name, Address and Phone number
